Medical Release Form

All players must have a medical release form on file and it needs to be updated on a yearly basis. We want to do all we can to ensure the safety of your player(s).

You may print it out and turn it in to your Manager or Team Mom, who will give it to one of our Board Members.

You may also fill out, sign, and return it to [email protected] - please include your player's name in the subject line!
